How to Request an Appointment
West Cancer Center schedules new patients primarily by referral from other physicians. If you would like to schedule an appointment at West Cancer Center, please have your primary care physician or referring specialist submit the Physician Referral form located below.
For physicians that would like to refer a patient via email or fax, please complete the New Patient Referral Form attached here and email to [email protected] or fax this information to 901.322.2940. For further assistance or additional questions, please contact 901.683.0055, Option 2, 1.

A second opinion can give you the confidence you need to make informed decisions about your care. The goal of a second opinion consult is to make sure you understand your diagnosis and all of the treatment options available. Whether you are unsatisfied with your current treatment or you just need extra clarity and reassurance, our providers at West Cancer Center are happy to help. New Patient Paperwork
Prior to your first visit, please complete the new patient forms ahead of time to ensure a smooth registration process on the day of your appointment. Failure to do so could delay your appointment by as much as one hour.
Patients should print your new patient paperwork here for submission upon arrival to your appointment. We will also mail you a printed copy once your appointment is scheduled. Should you need assistance completing these forms, please arrive to your appointment 30 minutes early so that one of our concierge associates is able to aid you with your packet.
What to Bring During Your Visit
Upon your visit to West Cancer Center, we recommend that you bring the following materials to expedite and ease your time with us
Please bring all current health insurance identification card, including secondary insurance cards. If your insurance carrier requires a referral, please make sure your physician has sent the required paperwork prior to the day of your appointment.

RX CARD, if you have one
Please bring any and all current prescription insurance cards if separate from your primary insurance policy.
LAB AND RADIOLOGY REPORTS
We make every effort to collect all necessary lab and radiology reports ahead of your appointment. Please bring any copies of lab reports, radiology reports, films, slides, or CDs you may have received from other physicians.
If you are scheduled for Mammography, you MUST bring copies of all prior mammograms or sonograms or your appointment will need to be rescheduled. Reports do not provide enough information for us to provide a thorough consultation.
LIST OF ALLERGIES
It is important for your health care team to be aware of any allergies you may have. Please bring a list of all known allergies with corresponding reactions to your first appointment.
LIST OF PHYSICIANS
Please bring a list of all the names, addresses, and phone numbers of any doctor you have seen in the past year. This includes your primary care physician, cancer specialist, or any other specialist.
LIST OF MEDICATIONS (or the bottles)
Please bring a list (or the bottles) of any medications that you are currently taking, including information on the dose, how often you take each medicine, and the name of the doctor who prescribed the medication. This list should include any over the counter medications, such as Tylenol, allergy pills, herbal supplements or vitamins. We suggest you keep a copy of your medications for future reference.
FAMILY MEMBER OR FRIEND
We encourage you to bring a friend or family member with you to your appointments. They may take notes or just provide support for you during your appointments.
What to Expect
When you enter our building, first, please check in for your appointment. There are directional signs located inside the front entrance identifying our check in desks. We have designated patient drop off areas located at the front door with wheelchairs available upon request. If you need additional assistance or directions, look for our staff or volunteers as they are happy to help!
PATIENT CARE MONITORS
After checking in, you will receive an electronic tablet, called a “PCM” or Patient Care Monitor. Please register yourself in the system. The PCM will then automatically start your survey consisting of symptoms to rate on a scale from 0-10, with 10 being the most severe. Please fill this out thoroughly and accurately. This survey helps our care team gather all necessary information to create your personalized care plan and address any new symptoms. The survey takes 5 minutes to complete and will need to be finished before each office visit begins. This survey is part of your medical record. Once completed, please return your tablet to our check-in desk.
Each West Cancer Center campus has patient ambassadors and volunteers available to answer any questions you may have, escort you to your appointment, serve snacks and refreshments during your visit, and offer their company during your wait time. Please do not hesitate to call on them for assistance.
BLOOD TESTS (IF REQUIRED)
Some patients may need simple blood tests performed before your appointment. If needed, our phlebotomists will contact you once you have checked in for your appointment.
CARE SUPPORT MEETING
If your appointment is cancer-related, you may meet with your dedicated Care Support Coordinator. Your Coordinator is here to provide you and your loved ones with proactive service, assist with any questions and concerns that may arise, and help address any barriers you may experience in your journey. Some of the barriers could be language, culture, transportation, financial assistance, cancer education. Our Care Support Coordinators work closely with your medical providers and serve as your liaison between visits to our clinic. They will remain a constant touchstone for you and your loved ones throughout your journey here at West Cancer Center.
HEALTHCARE TEAM MEETING
Once in the exam room, members of your healthcare team will conduct an assessment exam and ask general questions regarding your symptoms, allergies, and medications. Your doctor will review your medical history, perform a physical examination, and discuss any test results. The doctor will explain your treatment options and answer any questions you may have.
Your care plan is a combination of your doctor’s recommendations and your own healthcare decisions. Your trust in our care team is especially important to this process, so we want to make sure that we provide you with all the time you need to discuss these details with your doctor.
HOW LONG IS MY APPOINTMENT?
Because of the comprehensive nature of our services, it is not uncommon to have more than one appointment at our office on a given day, which could extend the time you spend with us. While this may mean extra time in the waiting room, it also means that you can feel confident that you are receiving the highest quality of individualized and comprehensive care.
